Before discharge, all follow up appointments will be arranged and documented in your baby’s discharge information.

The amount of follow up visits will depend on your baby’s condition and may change over time. If you don’t feel sure about any part of your baby’s care please speak to the discharge nurse or nurse in charge.

If you are expressing or bottle feeding at home with a different bottle/teat, it is helpful to bring this equipment in to the hospital so that your baby can start to get used to using them before they go home.

You may also like to bring in a wrap that smells of home for your baby’s cot. Once your baby arrives home, they will be comforted by familiar objects and smells.
